Simply complete the application form below with the details of your redemption and we will organise the payment directly to your provider.

Please note:

• Tax Invoices have to be made to your name c/o Member Benefits Australia
• Tax Invoices have to be in Australian Dollars
• An administration fee of 10% of the total invoice amount +GST (capped at $300 +GST) will be charged for each invoice submitted for My Choice payment.
